Action Lab Health Coaching, LLC ("we," "us," or "our") operates www.actionlabhealthcoaching.com and provides personal training, nutrition coaching, and mental fitness / mindset coaching services (the "Services"). This Privacy Policy explains what personal information we collect through the Site and in the course of working with your, how we use and store it, and the choices you have.
Where we operate: We are based in California and currently coach clients who are located in California. We do not market the Services to people in the European Economic Area, the United Kingdom, or Switzerland.
Section 1. Information We Collect
Information you give us.
Through the Site's contact form: first and last name, email address, and (optionally) phone number, plus any message you choose to write.
If you become a client: the information in the table below, depending on which Services you use, plus session notes and our communications with each other. We use health information only to coach you safely and to decide whether to recommend medical clearance or support from a licensed professional.
If you authorize us to contact a health care provider: the provider's name and contact details, and the information hat provider needs to identify you (such as your birth year or date of birth). We ask for this only at that point, only with your permission, and only to contact that provider.
Payment details: We take in-person payments with the PayPal POS app on our iPhone, using Tap to Pay, and we send invoices through PayPal. PayPal processes your card details and keeps a record of each transaction (amount, date and time, and card information). We do not store your full card number. We record your name and email address or phone number only if you ask for a receipt or an invoice.
Information collected automatically.
Browser type, device type, approximate location from IP address, pages viewed, and referring page, collected by our website host Wix through cookies and similar tools.
Your cookie choices: when you use our cookie banner, a record of your choice (what you accepted or declined, with the date and time) and technical information such as your IP address, so we can honor your choice and show we did.
Health information.
We collect health information only when you choose to complete our intake forms, and we use it only to coach you safely and effectively. We are a coaching business, not a healthcare provider, and we do not bill health insurance. As such, HIPAA does not govern the information you give us. We protect it as described in this policy.
Information about other people.
Our forms ask for an emergency contact, your health care providers, and your family's health history. Please share only what we ask for and only what you choose to. We use your emergency contact only if we believe you are having a medical emergency or we cannot reach you about a safety concern. We contact your health care providers only with your written permission, and you can withdraw that permission at any time.
Google Workspace file storage.
We deliver our forms through a Google Drive folder we create for you. Depending on the Services you choose, the folder may contain the PAR-Q+, our health history and intake questionnaires, and the Deep Health Self-Assessment, as online forms, fillable PDFs, or spreadsheets.
We share the folder only with the email address you give us, and only you and we have access. If that address is a Google account, you sign in to open it. If it is not, we use Google Workspace’s visitor sharing: Google emails you a verification code (a PIN) that you enter to open the folder, and access lasts seven days after each verification. To get back in, use the link in our original invitation and verify again. Google hosts the files as our service provider (see Section 3). Please don’t forward the invitation or share the folder with anyone.
We do not knowingly collect information about you from third parties other than the service providers listed in Section 3.
Section 2. How We Use Your Information

Tell you about other Services we offer that may fit your goals, in our sessions or, if you have opted in, by email.
We may use information you give us for one Service to coach you in another Service you join. We may also use your answers to suggest other Services of ours that may fit your goals in our sessions or, if you have opted in, by email. Our marketing emails never mention your individual health information, and we do not share your health information with advertisers or use it to target ads.
We do not sell your personal information, and we do not share it with third parties for their own advertising.
Section 3. Where We Store Your Information
Where and what is stored.
Wix (website host): Contact form submissions (name, email, optional phone number, and any additional information you choose to share) and, only if you accept analytics cookies, Site analytics.
Usercentrics (cookie consent banner): Your cookie choices, with date, time, and IP address.
Apple iCloud (backups of our iPhone): Backups of our phone, which may include PayPal app data, such as receipt contact details if you asked for a receipt
Google Workspace: Business email, calendar, and intake questionnaire responses and client records (session notes)in Google Forms and Drive.
Positive Intelligence® (PQ®). Your PQ® account, Saboteur Assessment results, and program progress, collected and held by PQ® under its own terms and privacy policy. We can view your program progress.
Our computer (disk encrypted). Copies of client files.
Paypal (point-of-sale payments). Payment and billing details, and receipt contact information if you ask for a receipt. We do not store payment information.
We do not sell your personal information, and we do not share it with third parties for their own advertising.
Section 4. How We Share Information
We share personal information only with:

Service providers listed in Section 3 that help us run the Site and Services.

Your care providers or other support services, only with your written permission. Support services may include:

emergency services or your emergency contact, if we believe you are in immediate danger.

legal authorities, if required by law or to protect someone's safety.

a successor, if the business is sold or transferred.

Our service providers include Wix (website hosting and analytics), Usercentrics (cookie consent), Google (email, file storage, and video), PayPal (payments and invoices), and Apple (phone backups). We do not sell your information or share it with advertisers.
Section 5. Our Use of AI Tools
General AI tools.
We use general-purpose AI tools (such as AI chat assistants and AI add-ons for spreadsheets and documents) as a thinking partner and drafting aid for our business, for example brainstorming program ideas, outlining content, and planning. We do NOT enter client or prospect names, contact details, intake forms, health records, assessment results, session notes, recordings, transcripts, or anything else that could identify you into those tools. When we use AI tools to work through a coaching scenario, it is a general or hypothetical scenario with identifying details removed. We do not use AI to transcribe or record sessions or any other interactions with you.
If these practices ever change, we will update this policy before they do.

Unsubscribe from marketing emails
We will verify your identity (usually by confirming the email or phone number on file) and respond within 45 days. We may keep information we are required to keep for legal, tax, insurance, or dispute-resolution reasons, and we will tell you if that applies.
Section 7. Cookies and Third-Party Tools
Our Site uses a cookie banner provided by Usercentrics. Until you choose otherwise, only essential cookies, which the Site needs to work, are active. Non-essential cookies, such as those used for analytics, stay off unless you accept them in the banner, and you can change your choice at any time by clicking on the blue icon with the fingerprint in the lower left corner. The Site’s analytics tool is Wix Analytics, and it runs only if you accept analytics cookies. We do not use advertising or marketing cookies, tracking pixels, or retargeting tools, and we do not track you across other websites. The banner itself loads from Usercentrics each time you visit and uses your IP address to show it to you. The Site does not currently respond to “Do Not Track” browser signals.
Section 8. Children
The Services are for adults. We do not knowingly collect personal information from anyone under 18. If you believe a child has sent us information, email sunny@actionlabhealthcoaching.com and we will delete it.
Section 9. How Long We Keep Information
Website inquiries that don't become clients: 12 months then deleted.
Client records: For as long as you are a client plus 4 years, then deleted or anonymized.
Section 10. Security
We protect information with two-step verification on our website, email, and cloud accounts, disk encryption on the computer where client files are stored, and access limited to the owner. No system is completely secure, and we cannot guarantee absolute security. If a breach affecting your information occurs, we will notify you as required by law.
Section 11. Third-Party Links
The Site may link to other websites. This policy does not cover them; please review their privacy policies.
Section 12. Changes to This Policy
We may update this policy. The "last Updated" date shows the latest revision, and we will notify clients by email of material changes to how we use their information.
